Orthodontic Retention is still the hot topic on everyone’s lips, reinforced by the Hold That Smile campaign launched by the BOS last September. As many studies show that teeth can relapse to their original positions in approximately 70% of orthodontic treatment cases (1), the million dollar question is; which is the most effective and suitable way to “hold that smile”?
We are all looking for something that is comfortable and suits each individual patient’s needs, yet offers a practical and simple protocol for all Orthodontic professionals. The advantages and challenges that accompany many retention systems on the market encompass many areas such as cost, patient ease of use, comfort, chair time, occlusal interferences and risk of relapse.
The popular choices
Vacuum formed removable retainers are popular. Offering fast turnaround on retainers where the model is poured and retainer is fabricated can be quick, easy and cost effective. However, its success lies in the hands of your patient; can you rely on them to wear it every night?
Popular braided wire retainers; and gold and stainless steel chain retainers eliminate the need for patients to remember to wear the appliance, and in addition to holding the position of the teeth they can give periodontal support or retention of a diastema if needed. A drawback to these methods is the often lengthy chair side appointment required for a fitting and the potential for failure once fitted. Approximately 1/2 of maxillary and 1/5 of mandibular multi-stranded lingual retainers fail during retention in some form, either through bond failure or wire breakage (2). Braided and wire retainers also may cause occlusal interference; hygiene complications; including difficulty in flossing; and torque forces causing unwanted tooth movement.
Why change to a digital solution?
Memotain is a bonded lingual retainer offering a revolutionary solution which tackles many of the challenges above. Memotain involves a CAD/CAM patent pending manufacturing process which laser cuts Memory-Metal Nitinol© wire to an individual patient’s anatomy. Made from Nickel Titanium, or NiTi for short, Memotain retains its position, yet still has the necessary flexibility required for everyday mastication forces. It is precisely laser cut out from a sheet of nickel titanium which eliminates any need to bend the material – a process still used in other systems that can create weaknesses and lead to fracture. After cutting, the wire is electropolished. This brightens, cleans, smooths and polishes – lessening the chance of microbial colonisation. During this process, the corners of the rectangular wire are also slightly rounded. With a thickness of 0.4mm by 0.4 mm, the wire possesses a low profile which means the patient is less likely to encounter occlusal complications or discomfort against the tongue. A simple fabrication process
The process is easy. Simply post your alginate or silicone impressions or send your digital files to the DB lab, Studio 8. Digital platforms including ITero, 3Shape and Carestream scanner (CS Connect) are recommended, along with the Memotain Lab form which can be downloaded from the DB Orthodontics website. The turnaround is ten days, but if you choose to review the position of the retainer, allow slightly longer.
